When the ground under the box is soft
Boxing out takes the driveway down to the depth your concreter has set. What's sitting at the bottom of it is whatever the block is made of, and on a filled or heavy clay site that can be softer than a base wants. You find that out with the box open, not before.
It's a normal thing rather than a disaster. It usually means going a bit deeper and putting rock back in, so it moves the number rather than the job. We'd rather stop and tell you than build a base on ground that won't hold one, so warn your concreter that the call might come.
